Annual Medical Information Form
*Please note: this form is a consolidated version of multiple forms used in the past. It replaces the Field Trip form, the Medical Info form and the Health History form used last year.
THIS FORM IS REQUIRED FOR ALL GREEN HILLS STUDENTS EACH YEAR.

Asthma Packet
To be submitted by the first day of school for any students requiring asthma medication(s).

Allergy/Anaphylaxis Packet
To be submitted by the first day of school for any students with anaphylactic allergies.

Physician Medication Order
Required for any prescription or over-the-counter medications given in school. Renewed annually. Exceptions include inhalers and epi-pens; there is different documentation required for those.
