PRESCHOOL REGISTRATION FORM

Once your child is admitted to our preschool program, please submit the documents listed below to complete the registration.

• Copy of Birth or Baptismal Certificate
• Immunization Record
• (3) documents for proof of residency (ie. utility bills, cell phone bill, lease, tax bill etc.)
• Copy of child custody agreement, if applicable.
