Welcome to Green Hills School
New Student Registration
Please submit the following information for registration:
- Registration form (click below)
- Three documents for proof of residency (ie: utility bills, lease, tax bill, etc)
- Copy of Birth or Baptismal Certificate
- Immunization Record
- Student Transfer Card from previous school (if entering from a NJ School)
- Copy of child custody agreement, if applicable
