Kindergarten Registration 2025-26
KINDERGARTEN REGISTRATION
Required Documents Needed for Registration:
- Students must be 5 years of age by 10/1/25
- Registration Form (See below)
- Copy of Birth Certificate
- Copy of Immunization Record - (Most recent record)
- 3 Documents for Proof of Residency (Utility Bills, Lease, etc.)
- Copy of child custody agreement if applicable
Forms to complete registration:

- Kindergarten Physical Examination Form -(Due to the nurse by June 30th) K-Physical Form
- Student Social Information Form - Social Information Form
- Annual Medical/Field Trip Form - Medical Form
- Student Health History Form - Health History Form
Forms for Parents to keep:

- Pick-Up Patrol - Information , Pick Up Patrol Directions
- NJ School Immunization Requirements - Requirements
- Notes from School Nurse - School Nurse Notes
