THIS FORM IS REQUIRED FOR ALL GREEN HILLS STUDENTS EACH YEAR.
To be submitted by the first day of school for any students requiring asthma medication(s).
To be submitted by the first day of school for any students with anaphylactic allergies.
Required for any prescription or over-the-counter medications given in school. Renewed annually. Exceptions include inhalers and epi-pens; there is different documentation required for those.
